Preparing for Disaster

I find it almost impossible to understand how BP could not have been prepared for what happened. Looking at a news illustration of the pipe coming up from the ocean floor to the drilling platform, I wondered how is it possible that BP engineers did not have a solution to a possible explosion? I’m certainly no expert, but when I look at those illustrations, one of my first questions is, “What would I do if that pipe broke?”  It should make managers everywhere wonder what disaster could possibly happen at their own workplace.  What is the obvious accident waiting to happen that they haven’t prepared for?
